Job summary We are excited to be recruiting to our Community Specialist Palliative Care Nursing Team for North Worcestershire. The service covers a wide area from Tenbury Wells in the west to the outskirts of Birmingham in the east. The service has two location, Princess of Wales Hospital in Bromsgrove and KEMP Hospice in Kidderminster. The advertised post is to be based at Bromsgrove, with an expectation of supporting the west side as clinical need requires. We provide a service 365 days a year from 9am to 5pm, with weekend and bank holidays covered by a rota which the successful applicant will join once they have settled into the team. You should be an effective team member willing to share experience and knowledge of palliative care to help support your colleagues as well as a willingness to learn and develop as the team strives to deliver excellent care. It is essential you can fulfil the travel requirements of the post. Main duties of the job The successful candidate will be working collaboratively with Palliative Care services across North Worcestershire including Palliative Medicine Consultants, Palliative Care in-patient Unit, Neighbourhood Teams, specialist palliative care services and specialist services within the neighbouring acute Trusts, Hospices and primary care colleagues to provide expert advice for patients with a life limiting diagnosis. The role will include caseload management, being a primary resource to health professionals in care settings giving advice for symptom control and advance care planning. You will attend and present patients to the weekly MDT meeting and actively support the GSF meetings held by primary care colleagues.
