As a General Practice Nurse, the post holder will support the delivery of Treatment Room Services, ensuring the delivery of safe and effective nursing care to the whole practice population.
They will work closely with the GPs and Senior Management Team (SMT) to deliver the practice priorities.
Clinically, the focus of the role is the delivery of cytology and immunisation targets, and having an evidence-based practice approach for management of patients with acute and chronic conditions.
You will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery and procedures and providing leadership and direction for the nursing team.
Main duties of the job
1. To help develop the nursing services offered by the practice.
2. To provide clinical leadership within the nursing team.
3. To manage the care of patients with long term health conditions. This may include organisational responsibility for specific disease areas.
4. To ensure the effective management of wounds, ensuring ongoing learning and development for the nursing team to improve rates of healing and patient outcomes.
5. To work with the nursing admin staff to construct and maintain robust call and recall systems for chronic disease, immunisations, and disease prevention care programmes.
6. To provide leadership and support in the relevant practice services e.g. LES, DES and other incentive schemes.
7. Ensure treatment room compliance and effectiveness (including stock control, ordering and vaccine/cold chain management).
8. Subject matter expert for infection control prevention (and responsible for audits and submissions).
9. Provide pastoral support for staff when required, particularly in relation to issues that affect their ability to work.
10. To set up robust nursing protocols where appropriate e.g., childhood immunisations to increase uptake rates.

This role may involve working across Reading Holybrook PCN practices, including: Broadstreet Health Centre, Milman Road & Kennet Surgery, Abbey Medical Centre.
