Social Care Practitioner
A vacancy has arisen for a part time Social Care Practitioner to join us in a busy, supportive and dynamic Adult Social Work Long Term Team. The team consists of a Service Manager, a Senior Practitioner, Social Workers and Social Care Practitioners and supporting adults over 18 and their carers, plus transition cases from the age of 14.
You will be based within the High Peak Adult Social Work Team and will be covering the High Peak South area. There is an expectation you will be office based for part of the week and to work from home.
Training and induction will be provided alongside meetings including regular management and peer supervision.
You will need to have experience and an understanding of assessing and reviewing service user needs in the community and residential settings. Working within budgets is essential and having knowledge of outcomes focussed when completing assessments and reviews would be beneficial.
You will be expected to work in multi-disciplinary partnership way with other professionals, service users and their families and carers.
You will create person-centred and strengths-based assessments and support plans and commission services to enable individuals to meet their outcomes, promoting a home-first culture. You will need the ability to travel and ideally have access to your own transport for which a mileage allowance is payable.
