The primary purpose of this role is to provide comprehensive social work support to patients with frailty and life-limiting conditions, as well as their families and carers. This involves advocating for patients' needs, ensuring they receive appropriate interventions and resources, and enhancing their quality of life during challenging times.
You will work collaboratively within a multidisciplinary team to deliver holistic care that respects the dignity and preferences of patients, while also addressing the emotional, social, and practical challenges they face.
By developing strategies and frameworks for effective service delivery, you will help create an environment that fosters well-being, safety, and access to essential hospice and external services.
Play a vital role in safeguarding vulnerable adults and children, implementing policies and training initiatives that promote safety and best practices within the hospice setting
