To provide specialist nursing advice for both primary and secondary care and work to prevent TB by vaccination, education and the monitoring of patients receiving care and treatment. In conjunction with UK Health Security Agency Staff, primary and secondary care this post implements preventative and infection control measures according to agreed protocols. To work towards increasing community access to support a reduction in health inequalities and increase quality of life for patients with respiratory conditions Communication The post holder will maintain verbal and written communication with patients relatives and carers and be able to maintain effective communication with external agencies including acute sector, primary care and the voluntary services. Communicating sensitive condition related information to patients and relatives requiring empathy and reassurance. To be proactive in raising awareness of TB and act as a resource for other health professionals and the wider community.The post holder will be expected to participate in relevant meetings and undertake teaching sessions to staff and patient groups People Management Take part in 1:1 meetings and appraisals in a timely manner Maintain own professional development and requirement to take part in appraisal and KSF process Line manage staff as required. Clinical and Practice Governance Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act. Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken regarding the Trust Information Governance and Information Security policies. The post holder must adhere to the Trust risk assessment and risk management processes The post holder must adhere to infection control policies and procedures. It is a condition of your employment that you are currently registered with the NMC, and it is your responsibility to maintain your professional registration. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust. The post holder must participate in clinical and other audits as required.
