Job overview
An exciting opportunity has arisen for a qualified nurse to join the Aylesbury Health Visiting Team based at Stoke Mandeville Hospital, working with families and carers of children under five. This post is full-time, working 37.5 hours a week and is a fixed term contract for 12 months with the possibility of extension for the right candidate. The successful candidate will support the Health Visitors to deliver the Healthy Child Programme, gaining and developing skills within a diverse local community, public health and the children’s agenda.
An induction programme will be provided for the successful candidate to support their transition into the community health visiting team. Candidates do not need to have experience of working within the health visiting service but do require knowledge and understanding of the current health visiting service and safeguarding practice.
The successful applicant may have the opportunity to apply for Health Visitor training. To meet the requirements of this post you are required to be able to drive between different community sites.
Main duties of the job
* Working with clients in a variety of community settings e.g. home, clinic, family centre, educational establishment etc.
* Undertake Family Health Needs Assessment as directed by the Health Visitor or School Nurse ensuring prompt feedback of all client contact (contemporaneously) to line manager.
* Working with the team to implement and prioritise planned programmes of health promotion, education and advice on all aspects of prevention of ill health to individuals, families and groups.
* Undertake child and or family health assessments, including screening, in line with the Healthy Child Programme and according to local guidelines in various community settings as required, reporting back on any ensuing health problems.

Education, Qualifications & Training
* Registered Nurse/Awaiting NMC PIN
* Evidence of further continued professional development
* Hold / be working towards an NMC approved mentoring qualification
Skills, Abilities & Knowledge
* Good verbal and written communication skills and ability to negotiate
* Ability to manage and prioritise own caseload with supervision
* Willingness to undergo training (e.g. medications)
