RMN or Social Worker (SW)

Spencer Clarke Group are looking for a RMN or Social Worker (SW) to join one of our public clients on a full-time basis, for a fixed term contract. As a Keyworker, you will be responsible for managing a caseload of individuals with a range of mental health conditions, including varying diagnoses and severity. Your role will involve conducting comprehensive mental health assessments, managing the treatment and support of service users, and working closely with a multi-disciplinary team to ensure that clients receive the highest standard of care.
What you'll do:
* Caseload Management: Provide high-quality care to a caseload of individuals with a variety of mental health conditions, ensuring they receive the necessary support and intervention.
* Mental Health Assessments: Undertake thorough mental health assessments to identify service users' needs and ensure that appropriate care plans are implemented.
* Community Mental Health Nursing: Apply your knowledge of community mental health nursing to assess, plan, and deliver care within the community setting.
* Medication Management: Demonstrate experience in managing and administering mental health medications, ensuring service users are supported with their medication needs.
* Duty Diary Management: Manage a duty diary efficiently, responding to urgent situations, and providing appropriate interventions in a timely manner.
* Multi-Disciplinary Team Collaboration: Work closely with other healthcare professionals, including psychiatrists, psychologists, and support workers, to ensure that service users' needs are met holistically.
* Transformation Model Knowledge: A strong understanding of the new transformation model of the CMHT is desirable, helping to shape and develop the service further.
What we're looking for:
* Registered Mental Health Nurse (RMN) or Social Worker (SW) Qualification
* Experience in Mental Health Assessments and Care Planning
* Knowledge of Community Mental Health Nursing
* Experience in Medication Management
* Strong communication and interpersonal skills
* Ability to manage a busy and varied caseload
* Experience with Duty Diary Management
* Familiarity with the RIO system or similar case management tools is desirable.
* Understanding of the new transformation model of CMHT and its implementation is desirable.
