Contract: Permanent, full time
Hours per week: up to 37.5 hours per week
Location of work: Primarily based at Chapel Street Medical Centre and our Mayfield Medical Centre branch site. This will include working a Saturday morning on a rota basis. You will be required to work across both sites.
We are looking to welcome a Full time Practice Nurse to work alongside our existing nursing team.
Main duties of the job
You will be fully proficient in Treatment Room duties including but not limited to child and adult injections and vaccinations, cervical screening, contraceptive reviews, and wound care.
About us
Chapel Street Medical Centre and our branch site Mayfield Medical Centre have a current list size of approx. 12,500 patients. Our main Chapel Street site is based in Spondon Village, Derby with our Mayfield branch site being based approx. 3 miles away in Chaddesden, Derby.
We have recently invested in a major refurbishment project of both our sites to provide a better environment for both staff and patients.
We are a multidisciplinary Practice utilising a wide range of skills. Our team consists of 5 Partners, 7 Salaried GP's, 5 Nurses, HCA, Management Team, Prescribing Team, Reception, Admin, Secretarial and PCN Staff.
We are an innovative team and have developed our own Signposting system to ensure patients are booked in with the most appropriate clinician.

MAIN PURPOSE AND SCOPE OF THE JOB
Provide general nursing services and contribute as a member of the multi-disciplinary team.
To work within NMC Code of Conduct and to agreed practice standards and protocols.
DUTIES AND KEY RESPONSIBILITIES
Practice Nurse responsibilities:
* Perform cervical screening and promote breast awareness
* Administer adult and paediatric injections
* Provide adult and child immunisations ensuring full achievement of targets
* Ensure safe storage, rotation, and disposal of vaccines and drugs
* Undertake venepuncture and electrocardiography
* Undertake wound care / management / dressing techniques
* Carry out ear examination and syringing
* Perform and interpret Spirometry
* Provide advice on lifestyle issues, assisting and supporting patients in changing their unhealthy behaviours and refer appropriately
* Participate in the smoking cessation service
* Deliver travel health care, including vaccinations and advice for patients
* Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local, referral procedures
* Work with safeguarding teams where relevant
* Ensure that current evidence-based approaches to patient care are standard practice
* Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries
* Provide a comprehensive family planning service
* Participate in audit outcomes of care against standards and initiate change as necessary ensuring follow-up audit cycle
* Deliver effective management of patients with Chronic Diseases as detailed by the Quality and Outcomes Framework, local quality schemes and national quality schemes
* Support the implementation of evidence-based guidelines in conjunction with the wider team
* Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working
* Actively participate in clinical and practice meetings
* Assist in the development of clinical protocols for the surgery and the team

Person Specification
Qualifications
* Nursing degree or other relevant nursing qualification granting professional license to practice primary care nursing
* Qualified in chronic disease management for Diabetes, Asthma, COPD and CHD/CVD
* Independent / supplementary prescribing qualification
* Coil and Implant qualification
* Venepuncture and electrocardiography
Experience
* Registered Nurse
* Experienced in vaccinations, injections, and immunisations
* Experienced in wound care management, dressing techniques
* Experienced in delivering chronic disease management
* Experienced in delivering cervical screening
* Experience in performing and interpreting spirometry
Aptitude, skills and competencies
* Must be a positive individual with a good sense of humour who is committed to team working
