Key Accountabilities: 1. Undertake holistic assessment of patient needs, using an appropriate assessment tool, recognising the physical, psychological and spiritual needs of the patients, to enable to the development of an appropriate and personalised care plan. 2. Provide evidence based, personalised, specialist palliative care to our patients, including continual assessment, planning, implementation and evaluation of evidence based, individualised patient care. Maintain accurate clinical observations of the patient and acting accordingly, giving clear feedback to patients and family whilst working collaboratively with the multi-professional team. 3. Co-ordinate and lead a shift as the appointed Nurse in Charge, allocating the IPU Team resource, including volunteers, to best meet the evolving needs of patients. Follow the Nurse in Charge handover, ensure adequate staff levels for all shifts, and subsequent reduction in staff levels if requirements reduce. 4. Communicate effectively with family members, providing information, support and advice in relation to the care of the patient, ensure patient wishes and needs are paramount. 5. Take on additional roles and responsibilities that reflect the changing nursing role in line with the NMC standards for Practice, to ensure efficient and effective care is provided to address the needs of the patient. Actively support service development. 6. Engage with clinical supervision to reflect on experiences, develop clinical skills, strategies for handling specific circumstances and enhance your emotional intelligence. 7. Ensure all your mandatory training is up to date. Engage in educational activity to develop professional development to meet the revalidation requirements in line with the NMC guidelines. Demonstrate clinical competency in line with the Hospice competency framework, ensuring your clinical practice is up to date. 8. Participate in quality initiatives through active engagement as a champion, audit activity, facilitating education, policy writing and other tasks delegated as required of you. 9. Report incidents or concerns in a timely manner using the Hospice incident reporting system. 10. Actively engage as a practice supervisor and/or practice assessor to develop professional skills of students, whilst providing effective feedback. 11. To ensure the co-ordination and discharge of patients using the Discharge Bundle. 12.To maintain a safe environment when caring for patients, ensuring infection control procedures are adhered to, and risk assessment to reduce the risk of patient falls.
