Advanced Clinical Pharmacist - Parenteral Nutrition (XR08)
We are looking for an ambitious, self-motivated and committed Pharmacist to work within Intestinal Failure/Nutrition Services. You will work as part of a large multi-disciplinary team who make a real difference to patient care and clinical research within one of the UK's leading Intestinal Failure/Nutrition Services.
The Multi-disciplinary Nutrition Teams support over 220 adult and 20 paediatric patients to receive Home Parenteral Nutrition as well as providing excellent care to in-patients needing intravenous nutritional support across the Trust. Referrals are received from across the region for Intestinal Failure patients and the team are well recognised for the high level of care, expertise and knowledge displayed across the whole MDT.
You must have excellent communication and interpersonal skills, be self-motivated, and be able to work on your own initiative as well as within a team. Main duties of the job
This will be a varied role and will see you completing many different tasks on a day-to-day basis. This will involve:
1. Participating in the Nutrition in-patient ward rounds, MDT meetings and out-patient clinics.
2. Prescribing and validating homecare prescriptions and liaising with various homecare companies to ensure home Parenteral Nutrition patients receive appropriate, timely supplies.
3. Working closely with the Consultant Pharmacist for Nutrition and the wider MDT to implement service improvements enabling the team to continue to provide excellent care to adult & paediatric patients.
4. Working closely with the Consultant Pharmacist Nutrition and wider team to support the training of all staff across the department as well as the Foundation Pharmacists, Pre-Registration Pharmacists.
5. There is a weekend, late night and Bank Holiday commitment on a rota basis.

Job responsibilities
JOB DIMENSIONS
1. Leads the highly specialised clinical pharmacy service to patients and the multidisciplinary team within a given clinical specialty. Ensures medicines are used appropriately, safely, and cost-effectively in accordance with Trust policy, standard operating procedures and medicine legislation.
2. Manage other pharmacists providing specialised clinical pharmacy service within own clinical specialty depending on size of area.
3. Works directly and regularly with consultants, senior nursing staff and managers within the clinical specialty.
4. Supervise, along with other members of their multidisciplinary team, the development and implementation of treatment protocols, and guidelines for use of drugs within the clinical area.
5. Responsible for training pharmacists, and other health care professionals, on aspects of drug use within the clinical area.
6. Provide a minimum of at least one 4-hour session of service cover in the dispensary or aseptic dispensary each week.
Knowledge, Skills and Experience Required
1. Masters qualification in Pharmacy (4 years) or equivalent qualification plus one year pre-registration experience.
2. Registrant of the General Pharmaceutical Council (GPhC).
3. Postgraduate diploma in Clinical Pharmacy or equivalent experience or qualification.
4. Non-medical prescriber status.
5. Substantial post-registration experience as a practising clinical pharmacist.

* Postgraduate diploma in clinical pharmacy or equivalent.
* Non-Medical Prescriber.
