The Community Based Options service is responsible for supporting adults with learning disabilities within the structure of a day service. We have sites in Stockton, Ragworth and Billingham.
As a Adult Development Facilitator you will be a named staff member for a number of service users you will be responsible for:
* Ensuring that service users plans are person centred
* Support the development of clients timetables in a person centred way
* Plan, co-ordinate and deliver activities at Day Options taking in to account individual needs of clients and their abilities.
* Monitor and review plans, timetables and process any other paperwork
* Produce and review risk assessments for service users
* Support service users to identify needs, explore interests and develop
* Offer clients new experiences that are available in Day Options
* Liaise with senior staff members, social workers, families and carers to ensure the best outcome for service users.
* The efficient operation of day services, including rotation of Support Workers across day services.
What will you need to be successful?
* NVQ level 3 or above in adult social care.
* Experience of working in a care setting.
* Good verbal, written and IT communication skills.
* Empathy and a passion to support people.
* Ideally, a full driving licence that’s valid to drive in the UK and access to a vehicle to undertake site visits.
