Nurse Care Manager/Social Worker (minimum 1 year post qualifying experience):

About you
You must be registered with the Nursing and Midwifery Council as a Learning Disability Nurse or Social Worker (registered with SW England) on commencement in post.
You will bring to the role:
* Knowledge and understanding of current learning disability practice and the role of a multi-disciplinary team, including statutory and voluntary agencies.
* Skills in effective assessment, planning, intervention and evaluation of work in supporting people with learning disabilities.
* Ability to build good working relationships with professional colleagues and partners and engage positively with individuals, families and carers.
* Experience of using a person-centred, holistic approach and creativity in problem solving and reflective practice techniques.

Job Description - Newly Qualified Nurse Care Manager/Newly Qualified Social Worker
Job purpose Under the general direction of a Team Manager and with support and mentoring from more senior care managers, using a person-centred approach provide a high quality and comprehensive service to learning disabled adults, their carers and community in accordance with organisational needs, policy, practices and within statutory guidelines.
Responsibilities
* Manage an allocated caseload and with supervision, regularly monitor, review and evaluate changes ensuring that services contribute effectively to the support plan.
* On the basis of assessed needs and risks, devise and implement service and support plans which correspond to the assessed needs of service users, their families and carers.
* Monitor and contribute to review of plans relating to assessed needs of service users, families and carers.
* Work within Safeguarding Framework to investigate and manage risks where individuals have experienced or likely to experience significant harm.
* Prepare and provide concise, informative reports, based on an analysis of complex evidence which can be used within the legal context as required.
* Maintain and update case notes, other records using management information systems to keep records up to date.
* Develop and maintain effective working relationships with professionals internally and externally ensuring integrated, holistic and multidisciplinary approach to care management and safeguarding.
* Plan and negotiate a range of appropriate services and interventions that meet the needs of service users within delegated levels of responsibility.
* Work within the prescribed Professional Capabilities Framework.
* Undertake assessments including safeguarding investigations deemed suitable for allocation to a newly qualified worker.
* Develop and maintain effective relationships with service users, their families and carers: provide advice and support: help resolve conflict, where appropriate under clear supervision.
* Participate in meetings and relevant forums.
* Organise work activities taking account of the need to prioritise tasks and responsibilities ensuring statutory legislation and service priorities are met.
* Maintain performance and develop practice and experience in line with personal targets as agreed with line manager through supervision and appraisal.

Qualifications: Appropriate professional qualification (Registered LD nurse or dual LD Nurse / Social worker qualification) and registered with the Nursing and Midwifery Council (NMC) or Social Work England upon commencement in post and maintain registration are essential.

* Able to assess needs of and work with individuals and groups using a person centred approach.
* Able to maintain accurate service user records, assessments and follow policy and procedure.
* Able to record, evidence and communicate using technology.
* Able to exercise individual judgement, assess risk and consult where and when appropriate.
* Able to make proactive use of Supervision to reflect critically on practice and explore different approaches to work and understand the boundaries of professional accountability.
* Knowledge of the range of theories and models for Social Work intervention with individuals, families, groups and communities.
* Knowledge of relevant legislation, guidance, policies and the ability to interpret and apply them consistently.
* Demonstrate an understanding of the importance of confidentiality and the needs and wishes of individuals, families and carers as appropriate.
