Through the National Mental Health Transformation, we have prided ourselves on being able to move away from care coordination and focus our time on developing an outstanding assessment and intervention-based Community Mental Health Service within Telford and Wrekin.
The Community Mental Health Support Worker post is an exciting opportunity to work alongside a multitude of disciplines including but not limited to Psychiatrists, Psychologists, Mental Health Nurses, Occupational Therapists, Social Workers, Peer Lived Experience Support Workers, and Pharmacists, to develop and provide recovery-focused interventions for those who are struggling with their mental health.
You will be given a manageable caseload focusing on Anxiety Management, Cognitive Behavioural Therapy for Insomnia, Relationship Building, Stabilisation, Groups, Graded Exposure, and promoting integration within the community, to name a few.
Main Duties of the Job
1. Development of therapeutic relationships with service users and their families
2. Working closely with the wider MDT and partner agencies to increase people's independence, working towards an improved quality of life
3. Highlighting any concerns about risk
4. Involvement in care planning with registered staff
5. Involvement in reviewing needs of service users
6. Delivering interventions to service users to support in their recovery

Person Specification
Qualifications
* Advanced Apprenticeship in (Mental) Health and Social Care or NVQ 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSEs or Functional Skills) or equivalent experience, i.e. experience that equips the person with the necessary knowledge, skill, and competence as outlined below
Experience
* Experience of working with people with Mental Health Needs
* Experience of working with people with Mental Health Needs as a member of a multi-disciplinary community team
* Experience of co-facilitating group-based interventions
Knowledge
* Knowledge and awareness of issues relating to mental health, access to mental health services, and mental health promotion
* Skill in undertaking physical health assessment including the measurement and recording of weight, height, blood pressure, ECG, smoking, diet, and drug and alcohol use
* Knowledge and skill in providing relevant physical health promotion advice
