What we ask from you:- Role Working in collaboration with the muti-disciplinary team and colleagues to:- -Promote independence and support patient centred care planning.-Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).-Work autonomously, in line with your scope of practice, competence and policies.-Handover patient information to the wider team, document patient information effectively.-Escalation of patients needs as appropriate. Qualifications and training Essential-Degree in nursing or equivalent, including NMC registration. Practice supervisor training or a commitment to work towards.Desirable- Completed a placement within a community service. Genuine desire to work in a community nursing team. You must hold a full UK driving license and have the ability to travel around the geographical patch.
