The Care Coordinator will play a crucial role in supporting vulnerable individuals by coordinating care for Learning Disabilities (LD), Frail and Palliative patients, managing safeguarding concerns, and facilitating multidisciplinary team (MDT) collaboration to improve outcomes for vulnerable families and adults. You will be a team player, as well as being self-motivated, enabling you to take on your own responsibilities. You will have the opportunity to take the lead on allocated projects/services and support the team to implement, monitor and review. The post holder will work alongside the team facilitating the coordination, preparation, and documentation of support for patients in Care, Nursing and LD homes, including both virtual and in-person ward rounds. You will work alongside the Safeguarding team maintaining the organisations safeguarding registers and searches, ensuring that the organisation is meeting its statutory obligations in terms of safeguarding to improve outcomes for vulnerable families and adults. This role combines responsibilities for Learning Disabilities, Frail and Palliative patients with safeguarding coordination, ensuring that care is compassionate, timely, and meets statutory obligations.
