Practice Nurse with Diabetes- Prescriber Desirable
An opportunity has arisen for an enthusiastic, professional, and experienced Practice Nurse ideally with advanced certification in delivering the management of diabetes.
Stourside Medical Practice is a forward-thinking 6000 patient GP practice in the centre of Halesowen. We are close to the West Midlands motorway network and on bus routes from Birmingham and Wolverhampton. We are based in a modern health centre with associated community services under one roof. We are a Training Practice and have an experienced team of clinical and non-clinical staff which include a paramedic, a First Contact Physio, and First Contact Mental Health worker.
We are looking for someone to work 1-3 days with the ideal days being Monday/Tuesday/Friday. Full days are preferred to half days. We pay an enhanced rate for extended access shifts which are currently available to work if desired by the successful candidate.
Main duties of the job
The successful candidate will need experience of working with diabetic patients whether that is in a hospital/community setting or within general practice. Experience in chronic disease management and asthma/COPD is desirable along with knowledge in other areas such as Cervical Screening and Childhood Immunisations.

We have an experienced team of salaried GPs and Nursing team with the practice actively involved in a thriving Primary Care Network with a variety of allied health professionals supporting the practice including Pharmacists, Social Prescribers, Physiotherapists, and Mental Health support workers. We are an approved Training Practice with a trainee in post.

Job responsibilities
* Managing and treating long-term conditions with an emphasis on our 400 diabetes patients.
* Supporting and managing the health needs of women presenting for HRT, contraception, and cervical cytology consultations.
* Implementing and participating in vaccination and immunisation programmes for both adults and children, including patients travelling abroad, Childhood Immunisations, Prostap, Zoladex, Inclisiran, Denosumab, RSV, shingles, influenza, Covid-19, and pneumococcal vaccination.
* Meeting the needs of patients presenting for wound care or suture removal.
* Hypertension review and management.
* Asthma and COPD reviews.
* Management of other chronic diseases.
* Raising safeguarding concerns when needed.
* To maintain a clean, safe environment, adhering to hygiene and infection control.
* Provide home visits for housebound patients needing support for their long-term conditions.
* Using a clinical protocol to file normal lab results.
* To select patients for and attend a quarterly diabetic MDT with a hospital consultant and the GP diabetic lead.
* Review of medical triage submissions.

Experience
* 5 years post-registration experience including experience of working in primary care or diabetes management in a hospital/community setting.

Qualifications
* Current NMC registration with an active PIN.
* Hold an accredited Diabetes qualification.
* COPD Diploma.
