We are seeking a Band 8a Lead ACP to join our team here at the Great Western Hospital.
As an Advanced Clinical Practitioner (ACP), the role involves working within Urgent Treatment Centre alongside a team of Emergency Practitioners. You will be central to the delivery of Urgent Care within the Great Western Hospitals NHS Foundation Trust. The post holder is required to deliver an advanced level of clinical practice working within local and nationally agreed protocols and guidelines.
The ACP will exercise independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of Urgent Care patients. They will work alongside Urgent Treatment Centre (UTC) practitioners, doctors, and Lead Consultant and support the clinical decision making within the unit. The post holder will promote the UTC both internally and externally whilst acting as an autonomous, accountable advanced practitioner.
Main duties of the job
Advanced Clinical Skills:
1. Undertake comprehensive clinical consultations with patients as an autonomous practitioner using own clinical judgement to assess, examine, diagnose, plan and evaluate care for patients presenting with an undifferentiated diagnosis with the Urgent care.
2. Perform advanced therapeutic interventions as required by the service using defined local competencies.
3. Ability to request and interpret radiology requests within the guidelines and protocols for the clinical area.
4. As an independent non-medical prescriber, the post holder will prescribe and review medications within the practitioner's competency and scope of practice for therapeutic effectiveness, appropriate to patients' needs and in accordance with evidence-based practice.
5. The postholder will lead and maintain a nursing culture which is open to change, new ideas, concepts and innovations.
6. Effectively lead when delegated and monitor the delivery of care to ensure the individual needs of the patients are met in a safe and timely manner.
7. Provide information to patients, relatives and carers regarding the treatment options assessments, investigations and follow up.
8. Lead when delegated and support the development, implementation and review of standards, policies, procedures and clinical guidelines.

Person Specification
Qualifications
* Registered Nurse/1st Degree
* Active registration of Nursing and Midwifery Council
* Masters degree in Advance practice/PG dip
* Teaching and Assessing qualification
* Advanced Life Support Instructor
Experience
* Extensive Experience at senior level in relevant field
* Relevant transferable skills
* Ability to demonstrate advanced clinical skills e.g. history taking, physical assessments and clerking of patients presenting with undifferentiated diagnoses
* Proven track record, demonstrable knowledge and skills of formal research experience
* Evidence of prior experience of change management within the health care setting
* Demonstrates effective leadership, organisational, time management and motivational skills
* Evidence of published work in relevant field
Skills
* Ability to identify training needs of self and others
* Ability to request relevant investigations and interpret findings
* Ability to apply critical thinking/independent judgement to identify solutions to variable problems
* Advanced theoretical and practical knowledge of a wide range of approaches to the management of the patient caseload, demonstrated by advanced clinical reasoning, specialist knowledge and experience
