An exciting opportunity has arisen to join the Leeds GP Confederations (the Confed) Neurodevelopment Support Service as Pathway Manager, on a secondment/fixed term basis until 31st March 2026. The Neurodevelopment Support Service provides support, advice and guidance to people with ADHD or suspected ADHD. This includes providing wrap around support, delivering therapeutic interventions and facilitating referrals for ADHD assessment as deemed clinically appropriate. As Pathway Manager, you will be responsible for the operational management of the service, providing leadership to the Neurodevelopment Support Team, including line management of the Pathway Coordinator and Navigation Team. You will be supported in your role by the Business & Operations Manager, the Head of Pathways and Clinical Service Development and the Clinical Lead for Neurodevelopment Services. The role is offered on a fixed term basis until the end of March 2026, with a salary of up to £43,000 per annum. Due to growing demands for ADHD Services, Leeds GP Confederation have been commissioned to develop and implement a Front Door single point of access for all patients seeking ADHD assessment in Leeds. The Pathway Manager will be key in developing and implementing these services and providing day to day support and oversight to the to the Neurodiversity Pathway Coordinator and the wider Neurodevelopmental Support Services Team. The post holder will work closely with the Business & Operations Manager, Head of Pathways & Clinical Service Development, the Clinical Lead for Neurodiversity and the wider Leeds GP Confederation leadership team to ensure performance and success of these workstreams. Risks are identified, assessed for urgency and appropriately escalated to the appropriate person or service when required. The Navigation Team work from the agreed work premises and contribute to the overall smooth running of the services. The Navigation Team develop collaborative wellbeing support plans with individuals with ADHD symptoms which promote choice and complement any medical treatment or advice. The Navigation Team discuss alternative providers of ADHD assessments enabling clinically appropriate and informed patient-choice as part of a joint management plan. In cases where it is decided to refer, all necessary clinical admin will be completed as per agreed protocols. The Navigation Team identify any barriers to people accessing support for their ADHD symptoms and work with individuals to overcome these. The Navigation Team use a range of techniques and interventions to support people with ADHD symptoms, to better understand themselves from a strengths and challenges perspective, to encourage a more positive mindset and ability for self-advocacy. The Navigation Team monitor and record outcomes within SystmOne or EMISweb using agreed templates and assessment tools and outcome measures. Build networks with local community resources such as activities and services that may have an impact on the health and wellbeing of people living with ADHD symptoms and support individuals to access these. Identify gaps in provision for people with ADHD symptoms within the current Health and Care Partnership to inform future service requirements. Develop multi-agency working across communities and voluntary and statutory organisations to improve health and address health inequality for people with ADHD symptoms. Gather and collate statistical and other information and data as required, to report on activity and outcomes and ensure effective qualitative and quantitative monitoring and evaluation of the service. Reflect on practice, and participate in team meetings, practice development forums and peer supervision within the service. Provide supervision to the Neurodiversity Care Coordinator and Service Support Officer as required. Ensure that client records and other information systems are completed accurately and within agreed timescales. Ensure that support undertaken is meaningful and respectful, promoting self-understanding and self-compassion, and is in line with the individual support plan.
