We are looking to appoint an enthusiastic Clinician with excellent leadership skills into this newly developed service within RDASH.
The Enhanced Care Home Team is a multidisciplinary service providing district nursing, physiotherapy, occupational therapy and mental health nursing support to care home residents within Doncaster. The Enhanced Care Home Team is embedded within the Home First Services and as such provides assessment, treatment and care support to patients in care home. The service provides short term assessment for patients discharged from hospital as part of a Continuing Healthcare assessment pathway along with long term care planning including the embedding of clinical MDT’s and involves partnership working across Doncaster Place.
As Team Lead you will provide l operational management within the Enhanced Care Home Team pathways. You will be taking the lead with the triaging, planning, prioritisation and review of all referrals into the services, problem solving with the team any potential bottlenecks or issues with meeting service delivery targets to optimise patient flow. You will lead with service development for all professions within the team.
We are looking for a highly experienced Band 6/7 Clinician with a working knowledge of intermediate care and / or care home care provision who has the vision regarding service delivery in line with Home
First and Enhanced Health in Care Home Service delivery. Experience in managing complex hospital discharges and community rehabilitation would be preferable.
• To provide support to the Service manager to meet the aims and objectives of Doncaster Care Group.
• To provide support to the area service manager in the effective management of District Nurses, Community Mental Health Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Assistants within the resources available.
• Reviewing staffing levels and skill mix as necessary.
• To be responsible for the leadership, mentorship and support of staff on a daily basis
• Manage and provide management supervision to District Nurses, Community Mental Health Nurses, Physiotherapists and Occupational Therapists. Additionally to provide Clinical Supervision to team members in line with own profession and ensure clinical supervision for all other team members is in place.
• Work within the infection control agenda to improve cleanliness
• Improving environments for patients
• Resolve problems for patients and their care/relative by building closer relationships
• Building partnerships with other partner Organisations building community strength in localities.
