The Roxton Practice has a vacancy for a further Practice Nurse to supplement an already large Nursing team through participation in the NHSE PCN Pilot Programme as part of Meridian Health Group Primary Care Network (PCN).
The Roxton Practice is a large GP Practice with currently approx. 34,000 patients. The Main site is at Immingham with Branch sites at Weelsby View, Grimsby and Pelham Crescent, Keelby.
Main duties of the job
Responsible for the delivery of Practice nursing services care to the practice population. This will include clinical duties, health promotion, health screening, long term condition management, contribution to development of Practice services and participation in clinical audit.
The candidate should ideally have a chronic disease specialism in either diabetes and/or cardiovascular disease or respiratory; or a keen interest in developing in long term management. Supported by the Senior Nurses, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.
Main duties to include
* BP monitoring
* Perform other routine laboratory tests
* Assist other clinical members when necessary
* Provide lifestyle advice to patients making any necessary referrals within the Practice
* All aspects of wound care
* Record ECGs
* Undertake other tasks delegated by senior nurse
* Administer injections including travel injections and baby vaccinations as per patient group direction
* Undertake pill checks, woman's health services
* Cervical cytology
* Long term condition management
About us
We currently have 16 GPs and 4 Clinical Pharmacists. We are a training Practice for GP Registrars, FY2 medical students, Student Nurses, Trainee Paramedics and Trainee Nursing Associates.
The Large Nursing team currently consists of 5 Advanced Nurse Practitioners, 1 Nurse Practitioner and 2 trainee Advanced Nurse Practitioners, 2 Urgent Care Nurses, 9 Practice Nurses, 1 Nurse Associate, 2 Trainee Nursing Associates, 11 Advanced Healthcare support workers.
We also have a community team comprising of 1 Advanced Nurse Practitioners, 2 Paramedics and 1 Advanced Healthcare support worker.
The clinical team is supported by a large team of Administration staff and care navigators.
The position available is to supplement our team to provide General Nursing treatment.

Clinical practice
* Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being.

Holidays
Holiday entitlement in a full year is five weeks. This is negotiable depending on experience and is paid at four or five weekly intervals to coincide with the last Friday of each calendar month.
