Herefordshire and Worcestershire Heath and CareTrust are looking to recruit a compassionate and experienced Advanced ClinicalPractitioner to join our team of ACPs working across 5 Community Hospitals, delivering medicalcare for patients from sub-acute care to general rehabilitation, andincluding specialist stroke services, palliative care and fractured neck offemur rehabilitation. Thisdiverse clinical case mix requires dedicated practitioners with comprehensiveassessment and medical management skills to deliver safe effective care. We arebuilding a team for the future and recognise the benefits of ACPs leadingpatient assessment, management and interventional therapies,supported by a team of GPs. A Band 8a position will be offered to candidateswho have completed an MSc in Advanced Practice and can demonstrate experienceworking at an advanced level within a relevant clinical environment. Pleasenote, whilst the post is advertised with a base at Malvern Community Hospital,this is a rotational post, and you may be required to cover other sites onoccasions. Main duties of the job
The successfulapplicant must possess the MSc in Advanced Clinical Practice, including AdvancedHealth Assessment skill at Level 7, and be a V300 non-medical prescriber. Youwill work autonomously to admit and assess individual patient needs, determinea differential diagnosis, request investigations, and initiate appropriateholistic, research-based treatments, within a patient centred clinicalmanagement plan, include independent prescribing of medications. You will beexpected to exercise advanced clinical expertise, and high levels of decisionmaking in clinical care, whilst working closely with Medical, Nursing andTherapy teams to ensure patients receive appropriate holistic care, andco-ordinate a safe discharge with minimal avoidable delays. Additionalresponsibilities will include monitoring and leading on improvements tostandards of care through, supervision of practice, clinical audit,evidence-based practice, facilitation of learning through teaching andsupporting professional colleagues, and demonstrate enhanced professionalleadership skills.
This is a full-timeposition, however a minimum of hours worked over 3 days would beconsidered.
