Kirklees Care Association (KirCA) is a not-for-profit organisation representing and supporting independent social care providers across Kirklees. KirCA is sector led and is involved in a range of different projects within the social care sector. This is a great opportunity to build your skills and knowledge and join our multi-disciplinary team.
We are involved in an exciting project to support the discharge system in hospitals across Kirklees, Calderdale and Wakefield.
Patients can sometimes wait too long for discharge from hospital resulting in a poor experience of the health and care system and even poorer outcomes. The use of an independent Trusted Assessor collating information for an assessment on behalf of a care home manager can reduce the numbers and waiting times of people awaiting discharge from hospital and help them to move from hospital to another setting speedily, effectively, and safely.
This role will require you to work collaboratively with partners across the social care and health system to deliver effective timely discharges to Care Homes across Kirklees, Calderdale and Wakefield.
To be eligible for this role, you must have been working across the social care and health system. The ideal candidate will have relevant experience and understanding of the issues faced by the care sector, with applications especially welcome from individuals with nursing, care home management or social work experience. You must have the ability to work collaboratively as part of a broader team, recognising the importance of integrated working and good communication skills.
