We have a 6 month secondment (covering long term sickness) post at 30 hours. The role is a Band 6 clinical nurse based in Y Bwthyn Princess of Wales Specialist Palliative Care unit. The position provides an outreach service to patients with palliative care needs across the hospital and community setting. The service provides support for the population of Bridgend, surrounding Valleys to the West to Porthcawl and patients who attend the hospital. This post is a fixed term/secondment for 6 months due to service need. Main duties of the job The position involves directly supporting and advising patients with palliative care needs in a hospital and community setting, predominately as part of a MDT team. The core hours are Monday to Friday 9-5pm but depending on existing experience or knowledge gained during the undertaking of the role participation in a 7 day roster with the same working hours which will require giving advice to other health care professionals, patients and families, potentially visiting patients in the hospital and community setting at weekends.
