Join us and be a part of an innovative team working towards improving mental health outcomes for young people Main duties of the job The Band 5 clinician will be responsible for developing and delivering a high standard of assessment and treatment for patients referred to the service. This includes managing an agreed clinical caseload, ensuring continuity of care through effective care coordination as needed. In collaboration with patients, carers, and clinical teams, the clinician will assess and provide evidence-based treatment programmes and discharge plans, utilising the Care Programme Approach (CPA) to ensure a comprehensive approach to patient care. The clinician will also plan and provide appropriate therapy services, using evidence-based practice, while adhering to defined resources. The role involves developing and maintaining relevant, evidence-based therapeutic activities tailored to the specific needs of the patient. Regular review and evaluation of treatment programs will be carried out in conjunction with patients, carers, and clinical team members to ensure the effectiveness of the care provided. The clinician will actively participate in clinical meetings, case conferences, and CPA meetings, facilitating the smooth transition of patients through various stages of their care, including care planning and therapy programs. Additionally, they will be responsible for compiling written reports as required and ensuring that patient documentation is accurate and up to date.
