Responsibilities
Provide personalized support to clients, facilitating their daily activities and promoting independence.
Develop and implement individualized support plans in consultation with clients and their families.
Assist clients in accessing community resources and services, ensuring they receive the assistance they need.
Monitor and evaluate clients’ progress towards their goals, adjusting support plans as necessary.
Deliver emotional support, actively listen to clients, and help them navigate their challenges.
Collaborate with healthcare professionals, social workers, and other stakeholders to provide comprehensive care.
Advocate for clients’ rights and needs, ensuring they are informed and empowered in their decisions.
Maintain accurate and up-to-date documentation of client interactions, progress, and any referrals made.
Requirements
Relevant qualification in social work, community services, or a related field.
Experience in a community support or care role is preferred.
Strong interpersonal and communication skills, both verbal and written.
Ability to work independently and as part of a team, demonstrating flexibility.
Empathy and a genuine desire to help others, with a focus on client-centered care.
Good problem-solving skills, able to think critically and adapt to changing situations.
Basic IT skills for record-keeping and communication purposes.
