We are pleased to announce an opportunity for a Discharge Matron to lead our OUH Discharge Service, the position will be across all Acute sites and Short Stay Hub Beds as part of the system integrated Transfer of Care Hub. We are looking for a dynamic and autonomous practitioner to work within team linking with internal and external partners. The Discharge Liaison Nurse Team and Liaison hub Team are multidisciplinary teams consisting of nurses, therapists, and administrative staff; working across the various sites providing expert assistance to the clinical specialties to support effective discharge planning. This team provides complex discharge planning support and guidance, expert liaison with external community service providers, the Local Authority, Community Trust and Voluntary sector to achieve a safe and timely discharge. The Team is dynamic, the position will offer the individual opportunities to develop enhanced skills in expanding knowledge in complex discharge management and coordination, and enhance their negotiation skills.
