An exciting opportunity has arisen to join a busy and passionate CMHT within the centre of Swansea as a CPN. The role involves working as part of a multidisciplinary team to undertake the role of Care Coordinator, delivering high-quality community mental health services under the framework of the Mental Health Measure (2010) Wales, while maintaining a Recovery ethos at the forefront of nursing care.
Main duties of the job
1. Assess, implement, and evaluate care and treatment plans for each individual on the caseload, in agreement with the service user and carers.
2. Work autonomously under the indirect supervision of a Band 7 registered nurse or team manager.
3. Collaborate with other health colleagues to deliver supportive services for those under the care of EIP.

Detailed job description and main responsibilities
The successful candidate will work autonomously and as part of a team, conducting mental health assessments and serving as a point of contact for existing service users in the absence of their care coordinator. The role may also involve facilitating key clinics within the CMHT, including physical health checks and medication administration in line with local policy.

Qualifications
* Registered Mental Health Nurse
* Evidence of post-registration study (e.g., postgraduate diploma) or equivalent registration experience.
Knowledge
* Demonstrate clinical knowledge and understanding of current legislation (Mental Health Act 1983, Mental Health (Wales) Measure 2010, Mental Capacity Act, Deprivation of Liberty).
* Good knowledge of current care and treatment for adults with mental health needs.
* Understanding of the Clinical Governance agenda.
Experience
* Experience working in multidisciplinary and multi-agency environments.
* Experience in risk assessment and management.
* Experience at Band 5 or above in a related clinical setting.
* Leadership experience within a team.
Skills
* Excellent communication and interpersonal skills.
* Ability to travel between sites and patient homes efficiently.
