Orchard Surgery Dereham are looking to recruit an experienced Nursing Associate or Healthcare Assistant to join our friendly, forward-thinking clinical team.
The successful candidate should be able to demonstrate high standards of care and have excellent interpersonal skills.
Salary is negotiable depending on experience.
Main duties of the job
To be responsible for the delivery of phlebotomy, long-term condition reviews, NHS health checks, and wound dressing, working as part of the practice's multidisciplinary team who provide care within their scope of practice to the entitled patient population.

Orchard Surgery is a purpose-built surgery in the centre of East Dereham. We currently have 11,200 patients registered with us and are undergoing refurbishment for more clinical rooms and to grow our patient list size.
We are currently rated as "Good" by the CQC and are a research-accredited practice.
We work closely with neighbouring practices to form Mid-Norfolk Primary Care Network.

Job responsibilities
The following are the core responsibilities of the practice nurse:
* Develop, implement and embed health promotion and well-being programmes.
* Implement and evaluate individual treatment plans for chronic disease patients.
* Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health.
* Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and NSF.
* Provide wound care and post-operative care to patients including clip and stitch removal.
* Request and process pathology results.
* Provide chronic disease clinics (Diabetes), delivering patient care as necessary, referring patients to secondary/specialist care as required.
* Provide Women's health (Cervical cytology, contraception, etc.).
* Ensure read codes are used effectively to ensure QOF optimisation.
* Support the team in dealing with clinical emergencies.
* Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients).
* Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
* Support the clinical team with all safeguarding matters, in accordance with local and national policies.
* Monitor and ensure the safe storage, rotation and disposal of medicaments.
* Support junior members of the nursing team, providing guidance when necessary.
* Participate in the review of significant and near-miss events applying a structured approach.
Person Specification
Qualifications
* Registered Nurse.
* Postgraduate diploma or degree (primary care).
Clinical Knowledge and Skills
* Wound care / removal of sutures and staples.
* ECGs.
* Requesting pathology tests and processing the results, advising patients accordingly.
* Diabetes.
* CHD.
* Immunisations.
* Women's health.
* Ability to record accurate clinical notes.
* Ability to work within own scope of practice and to understand when to refer to ANP/GP.
* Understand the requirement for PGDs and associated policy.
* Clear, polite telephone manner.
* Effective time management.
* Ability to follow clinical policy and procedure.
* Venepuncture.
* Knowledge of local and national public health issues.
* Experience with audit and able to lead audit programmes.
