The post holder is responsible for the delivery of nursing practice within socially vulnerable communities, ensuring that everyone is treated with dignity and their needs are met in a timely manner. The focus of the role is to bridge the gap of health inequalities and provide education to the communities and professionals regarding the social determinants affecting vulnerable groups.
As an autonomous practitioner, the post holder is responsible for the care delivered, demonstrating critical thinking, sensitivity, and skills in clinical decision-making in the management of patients.
The practitioner will work collaboratively and maintain liaison and cooperation with other statutory, voluntary, and contracting agencies to ensure an optimum level of care is provided, particularly for Asylum Seekers and vulnerable groups including:
1. Support the Senior Nurse in delivering and supporting effective care to patients.
2. Assist with managing the healthcare service across the socially vulnerable groups that we support.
3. Deputise for nursing colleagues during periods of absence.
4. Represent the primary and community care viewpoint at operational and strategic meetings when necessary.
5. Work closely with the wider multidisciplinary team to ensure safe and effective delivery of health care, including mental health and public health services.
6. Promote and utilize the audit cycle within clinical practice.
7. Support the development and implementation of healthcare policies and procedures.
8. Conduct initial assessments and complete the “All Wales Blue Book” for Asylum seekers living in Newport.
9. Provide cultural awareness training and address the needs of socially vulnerable groups to other disciplines, such as GP practice staff, mental health teams, and Accident and Emergency staff.
10. Collaborate with other disciplines in the assessment, planning, implementation, and evaluation of healthcare service provision to ensure a smooth patient pathway through the services.
