Key Duties: Safe and Effective Clinical Practice: Work as part of team to triage referrals to the unscheduled bleeding on HRT pathway Working as part of a multidisciplinary team, contribute to the assessment, planning, investigations, diagnosis and treatment needs for patients referred on the unscheduled bleeding on HRT pathway, within own competence, skill and qualifications Working as part of the multidisciplinary team, attend the unscheduled bleeding on HRT clinics, to contribute to undertaking the medical history, vaginal and speculum examinations, pipelle biopsies, venepuncture, requesting further investigations (such as blood tests and pelvic ultrasound scan) and, with direct / indirect supervision, arrange treatment plans Working as part of the multidisciplinary team, ensure that patients are followed up in a timely and effective manner throughout the pathway To act with indirect supervision as a specialist nurse by planning and organising own workload effectively and being accountable for the quality and standard of work performed Contribute to the development, implementation and evaluation of care for patients whose needs may be predictable or unpredictable, to ensure delivery of high standards of care To identify the requirements of the patient needs in elective / emergency situations and have the knowledge and skills to act accordingly To gain consent for any procedures / investigations undertaken and be aware of potential diminished mental capacity using mental capacity assessment tools if necessary Provision of telephone / email support / ad hoc telephone consultations with patients as required to maintain or progress treatment plans To act as a resource for staff regarding patients with unscheduled bleeding on HRT who attend as emergency attenders to the Emergency Gynaecology Assessment Unit or elsewhere across gynaecology services Be a contact/resource for GPs and other healthcare professionals across the ICB To establish and maintain effective communication with patients in relation to health care needs about difficult or complex matters. To work to overcome any problems in communication by use of various media to communicate difficult or complex information with empathy To demonstrate a knowledge of HRT, its effect and side effects providing written and verbal information, advice, counselling and support to patients and their families as needed Demonstrate a good understanding of the post-menopausal bleeding pathway, referring patients to this pathway when clinical needs indicate To signpost patients to other services as appropriate including formal referral on to other health care professionals as required To support and involve patients and family in planning individualised care, liaising with and coordinating other members of the multidisciplinary team in the planning and delivery of individualised care To be responsible for maintaining patients records within agreed standards to ensure the care management plan is communicated to patients and other members of the multidisciplinary team Adopt a positive approach to infection control, actively contributing to infection control policy and practice Contribute to ensuring MDT recommendations are acted upon in a timely manner Plan and organise own workload effectively and be accountable for the quality and standard of work performed To be fully conversant with Trust IPC policies and adhere to these, being a role model to other staff Management and Leadership Manage own time and workload effectively. Maintain patient records to ensure that relevant information is documented and accessible to other health care professionals as required, in keeping with the NMC Code To develop collaborative working relationships between disciplines to enable effective, timely investigation and treatment eg. ultrasound scan department To contribute to the effective use of physical, financial and human resources Contribute to the supervision and training of newly-registered and non-registered nursing staff, providing advice and support to ensure safe and effective care and that educational needs are met Contribute to discussions and influence change in practice underpinned by research that is evidence-based. This will include assisting the lead nurse for PMB in development of guidelines and policy Act as a team leader where appropriate ensuring effective teamwork and promoting healthy working relations Monitor and maintain own and others health, safety and security Participate in devising induction programs for newly appointed staff Ensure and open, reporting and learning culture exists for staff to report incidents and near misses for investigation, taking remedial action where required and share learning to prevent reoccurrence of such incidents To assist the department manager in monitoring staff performance when requested To inform the departmental manager / responsible consultant of staff failing to meet the required standard of performance, and taking appropriate action where required To assist the gynaecology matron / lead gynaecology nurse in the maintenance of day-to-day discipline within the department To signpost staff to relevant services in relation to personnel and professional matters Education and Continuing Professional Development To maintain competency and produce evidence demonstrating continued professional development, identifying with own line manager and consultant lead for the service, when further competency / development is required To consider membership with the British Society for Gynaecological Endoscopy (BSGE), attending and completing courses required by the Trust to ensure safe clinical development and competency including the BSGE Annual Scientific Meeting To be conversant and comply with Trust policies and regulations, ensure compliance with Trust mandatory and essential to role training To participate in personal / career development plan to maintain and develop attitudes, skills and knowledge facilitating personal and professional growth to maximise own contribution and quality of service delivery To support the personal and professional growth of new, registered and non-registered staff in order to maximise their contribution to and quality of service delivery To participate fully in the implementation of staff personal development plan to facilitate on-going development Quality / Research and Development To contribute to clinical audit of the PMB services and unscheduled bleeding pathway, develop a working knowledge of reporting system and quality initiatives within the field Demonstrate research awareness, participating in research in nursing / medical practice, clinical trials and equipment testing as appropriate To work within an ethical and legal framework utilising the Nursing & Midwifery Council Code NMC, national and local guidance, policies, procedures, standards and protocols organisation to promote safe and effective evidence-based practice and contribute to quality improvement Contribute to the formulation of patient information leaflets and clinical/departmental guidelines in keeping with evidence-based practice To contribute to changes within own area of work using evidence-based practice to improve the quality of care Report and investigate delegated incidents in relation to the PMB service using the DATIX reporting system Professional To always maintain a professional attitude, developing the role of the nurse in the best interest of the patient and profession in accordance with Nursing & Midwifery Council Code Act as a professional role model to all staff To seek appropriate professional clinical supervision as required This job description will be subject to discussion and periodic review and may subsequently be changed by mutual agreement
