Job responsibilities In conjunction with other team members, to take on the role of case coordinator for a designated caseload of clients with - - a learning disability.To be responsible, for the comprehensive assessment/analysis, planning, implementation of programmes of care, providing each client with an individual holistic package of care. -As case coordinator, responsibility for making evidence based assessment of clients physical, psychological, spiritual and social needs taking into account and respecting ethnicity, for a defined caseload.Ensure the timely collection and accurate recording of relevant information for client care. Record keeping must be maintained to best practice standards.Maintain contact with the client and family whilst they are in hospital or other forms of care to ensure continuity of care until discharge from caseload. Offer advice and support to carers whether paid or informal carers.To work in an autonomous capacity within the community on a daily basis responding to changing needs and risks within the boundaries of safe working practice.To work closely with Health Boards with regards to assessing clients needs against the criteria for CHC funding.In the role of case coordinator provide written and verbal reports and attend Mental Health Review Tribunals/Appeals as legislated under the Mental Health Act. To be conversant with clinical governance issues and the need to be applying these to practice and to be conversant with and adhere to divisional and Health Board policies.To coordinate and organise case conferences when appropriate with the MDT.To promote relationships by working collaboratively within the MDT and other agencies.To ensure effective lines of communication are maintained with the clients and their carers and all disciplines involved.To participate in caseload audit as per polices and implement appropriate standards of caseload management in conjunction with the team leader.To promote a learning environment for students and NVQ nurses. Act as mentor and assessor in order for learning objectives to be achieved.To supervise student nurses and unqualified staff, providing and effective support role.To participate in local training and development initiatives.
