To provide a safe and cost-effective medicines management pharmacy service to patients who are cared for by a designated Urgent Community Response (UCR) team within the Eastern Community Services Division. A location will be agreed and may include: Exeter, Exmouth, Sidmouth, Seaton, Axminster, Honiton, Ottery, Tiverton, Cullompton, Crediton, Okehampton, Moretonhampstead.
To clinically assess and review the medication-related needs of patients on the Urgent Community Response caseload.
To be a source of medicines advice to colleagues and key stakeholders with respect to medicines optimisation for patients to support integration across health and social care.
To promote self-administration of medication through focusing on adherence interventions for individual patients.

We are looking for an enthusiastic, motivated medicines management technician to join the Exeter Central and East Urgent Community Response team.
The successful candidate will provide a medicines management service to patients in their own homes including medicines reconciliation, assessment of patients own drugs and advice regarding medication adherence. You will work as part of a multidisciplinary team supporting patients newly discharged from hospital or who are acutely unwell being cared for in their own home. The team includes nurses, physiotherapists, occupational therapists and social care staff.
You will be supported by the Exeter Community Services Pharmacy Team who have developed the home visiting pharmacy service across Exeter and are valued members of the health and social care team. You will be offered full training and opportunities for development appropriate to your role and experience.
This is an excellent opportunity to work with a wide range of patients and healthcare professionals whilst developing your skills and experience in Pharmacy.
