1. Conduct thorough assessments of adults' needs under the Care Act 2014 and other relevant legislation.
2. Develop and implement personalized care plans aimed at promoting independence and community living.
3. Safeguard vulnerable adults, ensuring their safety and well-being through appropriate interventions.
4. Work collaboratively with multidisciplinary teams, families, and community services to provide holistic support.
5. Advocate for adults' rights and ensure they have access to the necessary resources, services, and benefits.
6. Regularly review and monitor care plans to adapt to changing needs and circumstances.
7. Ensure compliance with statutory responsibilities and best practices in safeguarding and care provision.
