The Neurological Community Team is looking for an enthusiastic Band 4 Rehabilitation Assistant to work within the stroke pathway.
The successful candidate will:
• Work within a supportive multidisciplinary team
• Hold your own caseload of patients who have suffered an acute stroke, providing assessment and treatment for the Patients rehabilitation journey.
• Implement treatment plans based around the Patients own goals.
• Be based at Queen Victoria Memorial Hospital within the beautiful Welwyn countryside, but working from home is encouraged
• Be part of a fully integrated multidisciplinary team which includes Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Nurses and Rehabilitation Assistants
• Have clear structures in place to support your CPD and to provide support and supervision
• Have a full valid driving license and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
As a Band 4 Rehabilitation Assistant you will be working within a multi-disciplinary team of health and social care professionals, providing appropriate assessments and rehabilitation to patients with neurological conditions.
Additional responsibilities include;
• To maintain patient treatment records using Systmone.
• To undertake a clinical caseload using knowledge and skills from theoretical and practical experience.
• To work without direct supervision on own caseload within a specified clinical area undertaking tasks delegated by a qualified clinician.
• To act as a first contact in certain situations.
• To participate in clinical supervision, and service-level education to ensure that clinical practice is maintained to a high standard
