An exciting opportunity for a 12 month secondment has arisen within the Uro-Oncology specialist nursing team. We are looking for an individual to develop the skills of a Urology nurse specialist to enhance and support urology cancer patients (prostate, bladder, testicular, renal and penile cancers) clinical pathways.
In collaboration with the Urologists and the existing team of urology nurses and the matron for the Elective Division:
1. Develop and co-ordinate the Urology Nursing services specifically for the Renal cancer patients for the Trust.
2. Develop and monitor standards of care within the Urology cancer services.
3. Promote education and training to all staff working within the service, hospital and community.
4. Coordinate and lead on the provision of nurse led services for patients with suspected and confirmed Urological cancers.
5. Provide a urology oncology nursing service in collaboration with the cancer service team ensuring the delivery of a high standard of holistic nursing care in accordance with the individual clinical needs and in a timely appropriate way within agreed cancer pathways and national target.

Main duties of the job
1. Coordinate, plan, implement and evaluate for patients with urological conditions nurse led clinical care under indirect supervision undertake the following treatments:
2. Support the existing urology nursing team members towards the management of all urology cancers.
3. 2ww electronic triage and appropriate management of cancer patients pathways.
4. Attend and contribute towards the local MDT and SMDT.
5. Support and manage nurse Led Independent clinics including BCG/Mito clinics, Nurse Led prostate clinics.
6. Support cancer patients throughout their journey including fast-track investigations, support during diagnosis and Breaking Bad News, explain treatment modalities and ensure patient centred approach/holistic care delivery/follow ups.
7. Ensure effective communication, teamwork, collaboration, innovation, continuous learning and evidence based practice for the safe and effective patient care.

The post holder will:
1. Deliver and promote evidence based practice and agreed care pathways.
2. Identify patients' physical, social and psychological needs (Holistic assessment) in order to refer appropriately to other agencies.
3. Support nurse led services for example Nurse led clinics, working with the clinical nurse specialist (CNS).
4. Visit patients in ward areas offering specialist advice and support.
5. Ensure the provision of an integrated quality service by maintaining effective communication with the multi-disciplinary team, community services and voluntary agencies.
6. Book new and follow up Urology cancer patients for MDT discussion as per prescribed MDT booking proforma in consultation with the clinical teams.
7. Attend weekly Urology MDT as required and to attend other body site MDTs in consultation with the CNS and lead clinician.
8. Attend Network Cancer Group meeting when CNS is not able to attend.
9. Support development of a patient centred service, within the multidisciplinary team, that provides specialist nursing care and support to patients and their families/carers, with Urology cancer.
10. Assess both the physical and psychological needs of the patient and act as a resource to all disciplines involved in the delivery of care to the patients.
11. Advise and communicate with other health professionals regarding the support and management of these patients.
12. Lead by example in promoting & delivering high standards of evidence based clinical treatment and sharing clinical expertise.
13. Maintain Urology database/record keeping of all cancer patients and to enter data onto Somerset Cancer Register.
14. Arrange and co-ordinate investigations in a timely manner on the patient pathway.
15. Maintain good communication with patients and relatives providing timely information at all stages of the patient pathway ensuring that individual needs are met and addressing any issues identified.
16. Act as the patients advocate, providing education and information in patient friendly formats, which enables the patient to make informed choices.
17. Enhance the patient experience through the provision of information and support throughout the patient journey.
18. Work flexibly to meet the needs of the Urology service.
19. Participate in clinical audits as required.
20. Support patient involvement in Urology research trials.
21. Understand national guidance in relation to Urology care.
