Job Title: Primary Health Care Nurse
As a member of the Primary Health Care Team, you will develop and deliver high-quality service to the Practice Population. Your role involves providing clinical assessment, diagnosis, treatment, and evaluation of care for patients within your competence framework.
Main Responsibilities:
1. Participate in chronic disease management clinics, immunization, cervical cytology, health promotion, and day-to-day duties carried out by the nursing team.
2. Work closely with the practice team to meet patient needs in line with Practice Objectives and National Guidelines.
You must have undergone fully recognized nurse training and have previous experience working in a primary care setting. You are required to work within the Code of Professional Conduct and other directives as defined by the NMC.
Key Requirements:
1. Excellent communication skills, both written and verbal.
2. Basic computer skills and experience of clinical systems, such as System One.
3. Ability to work as part of a team, organize, and prioritize your clinical caseload on a day-to-day basis.
4. Responsiveness to patient need, ensuring patient choice, and ease of access to services.
Personal Development:
The post holder is responsible for their own personal development, ensuring Continuous Professional Development remains a priority. You will maintain your professional registration by attending relevant courses and studying topics related to the post.
Main Clinical Duties:
1. Blood pressure monitoring and new patient checks, reporting any abnormalities.
2. Phlebotomy/Venepuncture, following health and safety policies regarding safe handling/disposal of sharps/needle stick policy.
3. Preparing samples for laboratory, urinalysis, and reporting abnormalities to the clinical team.
4. Wound care and dressings, basic recognition, and referral for specialist input.
5. Smoking Cessation, basic support, and referral for specialist support.
6. Alcohol support and referral for specialist support.
7. Health Checks/new patient checks, including height/weight/BMI, following in-house procedures.
8. Infection control, awareness of all policies and procedures regarding prevention and control of infection, keeping up-to-date with training.
9. Vaccinations & Immunisations.
10. Chronic Disease reviews for respiratory, diabetes, cardiovascular, learning disability, hypertension, dementia.
11. Smear taker/Cervical cytology and Breast Health Awareness.
12. Travel advice with NHS vaccinations and malaria advice.
13. Zoladex and prostap injections, B12 injections.
Please note that not all of these duties may be required as part of your role.
