We are looking for a Social Work Assistant to join our team at Cygnet Hospital Derby on a 6 month fixed term contract. Cygnet Hospital Derby provides low secure and mental health rehabilitation care based on the Recovery Model. Our integrated male care pathway includes Litchurch Ward, our low secure ward, and the Wyvern Unit, our mental health rehabilitation service. Alvaston Ward, our specialist female personality disorder service, is one of the leading services of its kind in the East and West Midlands. Service users have access to external secure courtyards, and therapy and leisure facilities, including a fully equipped gym, art room, IT classroom and therapy kitchen, which contribute to a comprehensive package of care. Job Summary The post holder carries out assigned tasks in support of, and supported by senior members of the Social Work Team or in their absence Clinical Manager or nominated Deputy. Contribute to the provision of a high quality care for patients and to uphold the positive image of Cygnet Health Care. To work as part of the Social Work Department Team. Successful completion of a 6 monthly probationary period or employment or role Key Duties Include To assist service users to access benefits to which they are entitled, supporting with appointments and ensuring benefits database is kept up to date. To attend all mandatory training sessions, keep up to date with all E-learning and attend allocated training to role including PMVA & BLS. To attend incidents on Unit to assist staff. To attend ward MDT handover at 9am as allocated by Head of social work or Social worker or Clinical Manager. To attend home assessments as required To attend ward rounds and provide feedback and communicate any required actions as allocated by the Head of social work or social worker To review benefits entitlements with service users, and support them to complete DWP forms as necessary. To complete care plan clinics across all wards as allocated by head of social work or social worker To attend morning meeting for Unit co-ordination as required and directed. To participate in the monthly safeguarding champions meeting and completed allocated actions To identify service users who may like to be registered on the electoral register and support this to happen (maintaining a list of those who are on the register). To support the Social Work department and carer’s champions in completing carer’s passports To be actively involved in assisting in the planning and implementation of Family and Friends Forums. To assist the Lead Social Worker to ensure that the department meets all the identified needs of the service users and (where appropriate) their families. To co-operate with all departments in ensuring a safe environment is maintained for all service users. At all times to ensure that the service user’s rights and their dignity are respected and maintained. To adhere to company policy and procedure. To undertake duties delegated by the Qualified Social Workers or Clinical Manager to ensure the efficient running of the department. To undertake any reasonable task. To promote constructive relationships with all Cygnet Healthcare colleagues. To maintain in confidence all medical records and information pertaining to Cygnet Hospital Derby and its service users according to Cygnet Healthcare policy and procedure. To maintain security policies and procedures at all times. To record by writing and electronically relevant information and comply with relevant policies. To ensure the highest quality of care delivered to service users via personal interactions and adhering to agreed care plans and risk assessments. To attend Inductions, Probation reviews, monthly supervision, appraisals as required (with the Lead Social Worker or nominated covering person). To under supervision prepare reports for MDT meetings (ward rounds) when required. At all times appear smart and professional in dress and demeanor. To ensure outlook diary is maintained to show all appointments and activities and to share this with Social Work colleagues. To maintain a log of all activities that is accessible to Social Work colleagues in event of sickness and absence. To maintain confidentiality as regards other Cygnet staff and Cygnet Healthcare activities. To participate in promotional activities as directed. To cover on in-patient wards to facilitate therapy activities or observations, general ward duties with other staff or in times of staff shortages. Attendance at weekends on occasions for support of Carer’s promotional activities /Carer assessments maybe required. To update Databases as instructed within Social Work team.
