Job Summary
We are looking for an enthusiastic Band 5 Community Staff Nurse to work with the OOH District Nursing Service 3 nights per week. You will join a team of nurses delivering high-quality care to patients who are housebound, caring for patients with long-term conditions and complex health needs, developing your skills and knowledge while working towards an integrated healthcare structure.
You should be able to demonstrate your ability to make clinical decisions to provide evidence-based practice and care planning in response to clinical assessments.
Main Duties of the Job
* The successful candidate will perform holistic assessments of patient nursing needs.
* Plan, assess, implement, and evaluate care delivery according to changing health needs utilizing prescribing skills.
* Ensure practice is evidence-based through undertaking audits and critically appraising literature to achieve a high standard of care.
* The post holder actively supports the District Nursing team by undertaking responsibility for a delegated caseload.
* Liaise with the District Nurse team leader to ensure information sharing in relation to patient care, changes to care plan, continuity of care, and reviewing complex care packages.
* Mentoring, teaching, and assessing.
* Liaise with all healthcare professionals and other agencies to ensure effective service delivery.
* Involve patients and carers in the planning and delivery of care.
* Maintain accurate records in accordance with NMC Standards.
* Participate in the health needs assessment of the practice population to identify areas of unmet need.
* Ability to work without direct supervision and to solve problems.
* Work on a rota basis to facilitate weekend working across the district nursing teams.
