PURPOSE OF THE ROLE: The post holder will undertake all Children's Continuing Care (CCC) and Continuing Healthcare (CHC) processes and assessments on individuals who may be eligible or who may already be in receipt of funding. They will provide contact with patients, carers and families. The post holder will be expected to work across children and adult provider services and develop strong links with clinical services staff across the trust, as well as colleagues from external organisation such as Social Services, Education, Acute Trusts, other ICBs, independent contractors and private care homes and providers. KEY RESPONSIBILTIES OF THE ROLE: To undertake Children's Continuing Care (CCC) and Continuing Healthcare (CHC) processes and assessments on individuals who may be eligible or who may already be in receipt of funding this will include those in receipt of fast track funding at end of life and those in receipt of a Personal Health Budget To case manage individuals in receipt of NHS funded care and support, reviewing and reassessing needs, the service provision and support required, this will include those in receipt of a Personal Health Budget. To provide and receive highly complex, sensitive and contentious information, communicating unwelcome financial decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism. This liaison is an integral part of the role and will take the form of regular contact by telephone, face to face meetings, written and electronic communication. To ensure effective timely communication and liaison with key stakeholders, including partner agencies, patients and their families, and care providers. To be actively involved in the PHB process including setting up PHBs, reviewing and monitoring of PHBS, and the development of Commissioning Care Plans for providers. Support the achievement of timely delivery of service, to work to local policies, systems and processes around CCC, CHC, Fast track as directed with reference to the relevant key national and local priorities To review any service, and care, commissioned in regards to quality and appropriateness to the individuals need, to ensure that the provider is delivering the requirements of the contract ensuring value for money in all matters relating to packages for continuing NHS healthcare. To ensure familiarity with the children and adult National Frameworks for funded NHS care and maintain consistency in the application of the national and local policies on eligibility for CCC and CHC. Be confident in presenting cases to Children's panel and to MDT meetings and making recommendations on eligibility and the care/support required. Responsible for providing first level contact for disputes/appeals/resolution meetings between partner agencies or with patients / families / legal bodies, for example following unwelcome financial decisions. To attend appeal/independent review panel meetings/ Choice & Equity panels and present cases as required. To be able to identify safeguarding and quality concerns, maintain full awareness and stay updated around child protection issues and alertness to potential vulnerable young adults and take appropriate action To be able to apply the Mental Capacity Act (2005) appropriately and have a sound understanding Deprivation of Liberty Safeguards (DoLs) (and Liberty Protection Safeguards) To actively participate in Best Interest Meetings, Safeguarding enquiries, Child In Need meetings, case conferences or other relevant meetings as necessary Support the Team Leaders and managers in investigating and responding to any complaints. Act as an expert CCC/CHC resource for young peoples complex health care and learning disabilities to health and social care workers, patients, families and other organisations and individuals as appropriate including assisting with the development and delivery of training within the CCC/CHC department and externally. Support and undertake audit and research relating to CCC /CHC activity To ensure compliance with all statutory and mandatory training. To adhere at all times to the service guidance, and work within established policies, procedures, protocols, and systems ensuring quality standards and performance as directed.
