We have an exciting opportunity for you to join an established Specialist palliative Care service in South West Herts based at BRE, Watford Herts.
This role is part time, 0.6 WTE.
You will be:
* A qualified nurse with appropriate skills and experience working in the palliative / End of life arena.
* A well-organised, flexible professional.
* An experienced practitioner with non-medical prescribing skills desirable.
* Compassionate to the needs of palliative and end of life care patients/carers.
* An understanding of the 4 organisations who make up the palliative care providers in this division.
* Used to working collaboratively as part of a multidisciplinary team.
* Have a wide range of IT skills and experience with use of Systmone.
* Knowledge and skills of community nursing.
* Triaging skills to ensure the best outcome for our patients.
Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Person Specification
Qualifications
* Registered Nurse with current UK NMC registration.
* First level Degree or undertaking First level Degree.
* Willingness to undertake further academic or practical study at a higher level for e.g. Master's degree.
* Suitable applicants will have demonstrable post registration experience working at a senior level in palliative or end of life care.
* Candidates should have attained an appropriate level of experience to fulfil the competencies required in palliative care or oncology.
* Teaching qualification or willing to undertake.
* Do Not Attempt Cardiopulmonary Resuscitation competencies.
* Advanced Communication Course.
* Extended Nurse prescribing Qualification.
Experience
* Community experience.
* Evidence of experience in a teaching and mentoring role.
* Experience of cross boundary working in partnership and collaboration with health, social and voluntary sectors.
* Knowledge of NMC code of professional conduct.
* Understanding of current political and policy context in Primary health Care.
* Knowledge of research processes.
* Knowledge of clinical governance and its implications.
* Knowledge of resource management.
* Up to date evidence of continued professional development and of reflective practice in a professional portfolio.
* Proven leadership skills in some aspect of service development.
* Experience of participation in research, audit and evaluation.
