The post holder will achieve this by working as an autonomous member of the Specialist School Nursing Team, supporting a defined caseload of children and young people aged 5-19 years with complex healthcare needs and disabilities within the community and school settings. The post holder will carry out clinical procedures in partnership with other professionals and agencies in order to meet identified care needs. The post holder will establish and maintain effective communication with children and young people aged 5-19 years, parents and carers, colleagues, special schools and the multidisciplinary teams in order to achieve a positive health status for the child and family. The post holder will be a registered Children’s Nurse and be responsible for children and young people who are allocated to them and will assess, manage, plan and deliver care, including being professionally and legally accountable To provide clinical practice leadership to the team members by acting as educator, facilitator, role model, mentor and preceptor. To assist and facilitate a flexible team approach in meeting the health needs of the local school age population working with members of the team to identify areas of service development which meet identified needs and government. To enable the team to develop and adapt in order to meet future health needs. To encourage and facilitate the team to be reflective practitioners and to deliver effective research and evidence-based services. To support the team to work towards achieving the service and clinical governance targets. 6 Working as part of a team of special school nurses supporting each other and ensuring the health needs of the children are met.
