We have an amazing opportunity for a discharge liaison and Hospital administrator to join our caring and support team at our Holybourne Hospital in Roehampton, an Adult Mental Health hospital for men and women with severe and complex mental health conditions.
To provide a comprehensive secretarial and administrative support service across the four wards. To ensure that across the hospital CPAs and Discharge Planning meetings are organised, managed and co-ordinated effectively. To ensure that the tracking system is adequately managed, and that MDT's are fully engaged with the process and external teams are communicated to and updated. Provide administration support across the four wards.
What you'll be working:
* 37.5 hours per week.
* Monday to Friday.
* 09:00 am to 17:00 pm.
What you'll be doing:
Administration
* To attend and provide minute taking for community meetings on all four wards
* To attend and provide minute taking for Team meetings for all four wards
* To attend and provide minute taking for the MAC meeting
* Prepare summaries for the wards to present at Clinical Governance meetings of the monthly data (incidents analysis, risks, good practice etc) and prepare summaries of the key themes coming from the community meetings
* Maintain the electronic actions logs for community meetings
* To take part in the rota covering reception duty to cover breaks and/or absence
* Completion of core information for all patients on care notes system
* Set up weekly ward round meetings.
* Preparing and sending out of weekly ward round reports to external trusts
* Sending out invitations and organizing patient review meetings with professionals and parents/ carers
* SWLSTG spreadsheet daily updates EDD.
* Attendance spreadsheet to be kept of all invited attendees at ward rounds.
* Covering the MHA Administrator when required
* Provide administration support to the wards when required.
* To complete any reasonable management requests regarding Hospital Administrator.
CPA co-ordination and administration
* Ensure procedures and practices are in place to ensure that clinical documents are available pre and post CPA's and Discharge meetings.
* To work closely with the whole staff team, specifically the Responsible Clinician and MDT's, acting as a point of liaison between the service and the external stakeholders in relation to the co-ordination of CPA and Discharge meetings.
* To be accessible to stakeholders internally and externally when organising and preparing CPA's and Discharge Meetings across the wards and organising the MDT's availability.
* To support the responsible clinicians in ensuring that CPA and Discharge information is adequately prepared and circulated pre and post meetings within required timescales
* To ensure that on admission patients have their CPA meeting and initial discharge planning meetings in place. Dates should be agreed with the MDT in advance and then circulated to external stakeholders ensuring that there are clear pre-planned diary invites for all future meetings.
* To co-ordinate attendance and documents for all CPA's and Discharge Meetings across the hospital
* To maintain a database of CPA and Discharge activity within the service.
* To act as a point of contact for professionals to offer information as required on the CPA/Discharge process, liaise with case managers, families/carers, advocates and any others as required in association with the CPA/Discharge process.
* To assist in the organisation/preparation for regulatory visits and in the response to reports, as requested by your Manager.
* Action paperwork for the transfer of patients out of the service ensuring that CPA and Discharge documentation is included in clinical documentation
* Act as a point of liaison with external agencies and professionals in line with your responsibilities as CPA and Discharge Coordinator
* To attend and provide CPA information to the Hospitals Clinical Governance meetings to include, no of patients compliant, operational issues, and CPA effectiveness.
