As a Community Practitioner, you will have a genuine interest in collaborative working with adults with severe and enduring mental health needs.
As a registered professional, this is an ideal opportunity for someone who wants to develop their community clinical skills. As a core member of the team, you will also be responsible for your own caseload, providing ongoing assessment, care coordination as a Key Worker and delivering evidence based intervention in the community. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work flexibly and collaboratively with patients, families/carers and a range of statutory organisations.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and Advanced Practitioners.
We aim to attract and retain a diverse, talented and committed workforce, who are caring and compassionate, and therefore able to meet the demands of the modern NHS now and in the future. In return, we can offer a dynamic working environment in which to build a career.
We will offer professional and development training opportunities.
