We have an exciting opportunity for an Advanced Nurse Practitioner to join our Clinical Hub team within our West Midlands Region.
In this role, you will have the opportunity to bring your advanced nursing skills to an organisation dedicated to making a difference to peoples lives on a daily basis.
The Clinical Hub is a brand-new innovation aimed to enhance the clinical support provided to our prisons and patient groups.
As a Registered Professional youll be an integral part of the Clinical Hub team by bringing your advanced clinical skills, Leadership, knowledge and passion for care. The Clinical Hub team will provide advanced clinical service virtually and face to face to HMP Oakwood, HMP Brinsford, HMP Featherstone and HMP Stafford. The team will consist of an Advanced Clinical Practitioner, Specialist Nurses and Healthcare Assistants.
This position is full time, working Mon to Fri.
Youll receive an annual salary up to £60,000 per annum - DOE

Main duties of the job
As an Advanced Clinical Practitioner, your responsibilities will vary. You will
1. Complete Comprehensive Geriatric Assessments (CGA), diagnosis, treatment and evaluation of care.
2. Communicate and work collaboratively with the interdisciplinary teams across sites to supporting the delivery of the Frailty framework.
3. Provide clinical education, leadership and upskill the local workforce.

Qualifications & Experience required
11. Registered Nurse Adult, Mental Health with current NMC registration / Registered Allied Healthcare Professional with current HCPC registration.
12. Independent / supplementary prescriber qualification.
13. MSc in Advanced Practice / relevant practice experience.
14. Completion of British Geriatric Society Tier 3 Frailty or willing to work towards.

As an Advanced Clinical Practitioner you need expertclinical skills and demonstrate judgement in assessing and managing wideranging and complex patient problems.You will provide clinical leadership for the mental health team. Thismulti-disciplinary team is made up of a variety ofregistered and non-registered professionals including; mental health, andsubstance misuse practitioners. You will act as a rolemodel ensuring professional appearance and maintaining professional standardsat all times.
Supporting senior clinicians on site you will be part ofthe daily leadership team, providing clinical input for all patients. As aclinical expert in your field, you will work with the team to ensure; patientsafety, the management of complex cases and ensure the most up to datetreatment methodology is applied to patient care.
You will also;
Provide o utstanding evidencebased care, listening and acting on patient feedback. Ensuring that care ispersonalised and informed by what matters to them. You will also provide assessment,diagnostics, treatment and continued management of patients with acute and longterm medical conditions.
Deliver non-judgemental care by leading the team to deliver carethat is safe, high quality, patient centred whilst meeting CQC requirementsfollowing our policies, procedures and always considering safeguarding.
