Outreach Support Worker / Coach
Salary: £22,932pa (pro-rata based on 35 hours per week) + Essential Car User Allowance £1,000pa (pro-rata)

Hours: 20 per week (This includes 1 weekend in 5 and shifts across the week. The shifts are covered between the hours of 9.00am – 5pm Monday to Sunday)

We’re looking for individuals who can help enable clients to live independently in accommodation suitable to their needs and to maintain their home in the community. You'll assist them in building skills that support access to work, training, education, and community-based activities. Additionally, you’ll support clients in maintaining financial inclusion, promoting healthy lifestyles, and engaging in meaningful activities.
In this role, you will be responsible for:
1. Using techniques such as coaching and strengths-based approaches to enable clients to develop emotional resilience, overcome barriers, and take control of their lives.
2. Liaising with external agencies and raising any safeguarding issues in an appropriate and timely manner.
3. Completing the Positive Behaviour plan, Support plan, Essential profile, and Risk assessment for the customers.
4. Managing workload and client numbers based on the needs of the clients and demand for aspects of the service.
5. Supporting the project manager to ensure that all aspects of housing management, housing-related support, and enablement support are carried out to the highest level.
6. Working as part of a team within supported housing and outreach services, providing high-quality, personalised housing-related support and enablement support to young adult clients on an outreach basis with a diagnosis of ADHD and ASD and outreach clients with substance misuse issues.
We need people who are:
1. Knowledgeable about the needs of complex and vulnerable adults.
2. Understanding of issues impacting clients' lives: behaviour management, understanding diagnoses, leaving care, and mental health.
3. Experienced in working in a supported housing environment or relevant social care setting.
4. Experienced in working with young people and vulnerable adults with complex support needs.
5. Experienced in collaborating with statutory agencies and voluntary support services.
