The team is based at Hall Court in Telford and provides mental health assessment, treatment and support to people in Telford & Wrekin. As an integral team in the Urgent Care Pathway, we work 24 hours a day, 7 days a week, with people who are experiencing a mental health crisis, require extra support alongside their regular support network, or can receive the same treatment at home that they would in a Mental Health Hospital.
We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators, and Business Support Staff. Our diverse skills and interests create a strong and responsive team.
Main duties of the job
Working in a multi-disciplinary team and in partnership with internal and external agencies, you will:
1. Provide specialist assessment and care planning in conjunction with service users and carers.
2. Evaluate treatment plans and undertake home treatment.
3. Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues.
4. Co-ordinate the work of the Team.

Job responsibilities
Assessing mental health crises and delivering safe and effective home treatment for an identified client group in accordance with that person’s wellbeing and recovery action plan where one exists, or creating such a plan where not.

Person Specification
Qualifications
* Be a Registered Mental-health Nurse as defined by the Nursing and Midwifery Council and be active on their register.
Experience
* Previous experience of working in the community.
