Job summary The Continuing Healthcare andcomplex health Team is looking to appoint a small team of 4 Case Workers tosupport the Continuing Healthcare Team Nurse Assessors and Complex CaseManagers in the ongoing case management of those individuals in receipt ofhealth funding. This is an exciting new role within the Team which is a busyoperational team within Gloucestershire ICB. Main duties of the job The Case Worker team willwork alongside the team of Nurse Assessors and Complex Case Managers within arange of settings and will undertake the non-clinical case management of thosein receipt of NHS Continuing Healthcare (CHC), Funded Nursing Care (FNC) andother sources of health funding. This will include liaising with placements,monitoring of care provision and changes to presentation and providing qualityassurance. The Case Worker Team will support the clinical staff to ensure thecare provision meets the needs of an individual, maintain accurate records, andsupport all aspects of work for those individuals in receipt of health funding.
