We are looking for a dedicated Discharge to Access (D2A) Care Assistant to join our team in Somerset and the surrounding areas. Access to your own transport is essential for this role. As a D2A Care Assistant, you will play an integral role in supporting individuals with their rehabilitation needs, providing care in the comfort of their own home, and working alongside a dedicated team to ensure the best outcomes for every customer. Key Responsibilities Provide compassionate, reliable support tailored to each customer's individual rehabilitation plan. Collaborate with healthcare teams, including occupational therapists and hospital discharge teams, to assess and meet the rehabilitation needs of each customer. Respect the dignity of every customer, ensuring care is provided with their informed consent. Monitor customer progress, ensuring goals are met while reporting progress in a professional and timely manner. Ensure clear communication within the team and with other professionals to support the customer's rehabilitation journey. Duties: Work closely with occupational therapists to design person-centred rehabilitation plans. Assist customers with daily tasks including personal care, medication management, meal planning and preparation, light housework, and more. Use aids and equipment safely, always respecting the dignity and preferences of the customer. Encourage participation in social, physical, and mental activities to promote well-being and independence. Record and report all relevant information regarding customer care, including changes in condition and any safeguarding matters. Maintain confidentiality regarding customer and family information, ensuring their privacy is respected at all times. Salary & Benefits: Competitive annual salary of £27,040 per annum. Travel allowance of 35p per mile. Full training provided, ensuring you are well-equipped to deliver excellent care. Overtime opportunities available. A comprehensive support network to help you thrive in your role. 40 hours per week, with a rota that is discussed during the interview. This will include a mix of early and late shifts, as well as working every other weekend. Essential Qualities: A compassionate approach to care, with excellent communication skills.
