The Digital and Transformation Lead will work closely with clinical and operational teams to drive service improvements that enhance patient care, improve staff and patient experiences, and make healthcare delivery more efficient.
This role involves guiding change efforts in alignment with Integrated Care System (ICS) strategies, focusing on digital and operational enhancements.

Job responsibilities
1. Transformation Planning and Execution
2. Develop a strategy for service improvements and work with Primary Care Network (PCN) staff to identify priorities.
3. Collaborate with PCN member practices to understand existing service models, identify challenges, and implement solutions to enhance patient care and operational efficiency.
4. Establish and maintain relationships with ICS and other partners to gain support for transformation plans, including digital initiatives.
5. Improvement and Innovation
6. Lead business and service improvements by identifying best practices and implementing changes that benefit both patients and staff.
7. Develop and manage a transformation plan that identifies dependencies, manages risks, and outlines resource requirements.
8. Support staff training and development to ensure effective use of new processes and technologies.
9. Data Management and Analysis
10. Use data to drive decision-making, assess impacts of changes, and track improvement trends over time.
11. Work with the ICS to identify relevant data sets and standardize measures for assessing progress.
12. Present data insights to PCN staff to improve understanding of service challenges and performance.
13. Communication and Engagement
14. Build strong relationships with stakeholders across the PCN and ICS to foster collaboration and secure buy-in for change initiatives.
15. Ensure changes align with clinical priorities and have the support of clinical leaders.
16. Share learning and best practices across the PCN and engage staff in improvement initiatives.
17. Policy and Service Development
18. Stay informed of relevant health policies and translate them into local strategies.
19. Work with the ICS to develop and implement primary care transformation plans, including digital strategies.
20. Help practices adapt policies to local needs, ensuring initiatives align with PCN priorities.
Person Specification
Experience Essential
1. Experience in general practice with a strong understanding of healthcare processes and systems.
2. Proven success in leading change and improvement projects.
3. Strong communication skills and the ability to build trusted relationships with a range of stakeholders.
4. Proficiency in program management, including translating strategic goals into actionable plans.
5. Data literacy, with the ability to use and present data to drive improvements.
6. A commitment to promoting diversity, inclusion, and patient-focused service enhancements.
Qualifications Essential
1. Relevant experience in business/service improvement, ideally in healthcare.
2. Familiarity with digital healthcare systems and how they can support transformation.
