Job summary Fareham and Portchester Primary Care Network - PCN is looking for a motivated Paramedic, Advanced Nurse Practitioner - ANP, Advanced Clinical Practitioner - ACP, or an experienced Nurse working towards a Level 7 qualification to join our dynamic and expanding same-day urgent care team and home visiting team. This role is integral to our commitment to delivering high-quality urgent care across four practices, focusing on acutely unwell patients, as well as offering a unique opportunity to work a network of both medical and non-medical professionals to deliver Coordinated, holistic care. Main duties of the job Key Responsibilities Patient Care Provide urgent care through telephone triaging, in-practice consultations, and home visits. Independent and Collaborative Work Ability to work autonomously during home visits and collaboratively within a practice team. Team Development Participate in and contribute to training, mentoring, and peer support initiatives to promote team development. Collaboration Link with doctors and other professionals during triage, home visits, and multi-disciplinary team (MDT) meetings to deliver patient-centred care. Holistic Approach Engage in proactive care by combining medical opinions, medication reviews, mobility assessments, and health connections, with a focus on continuity and follow-up. Integrated Patient Care Work closely with a multidisciplinary team to coordinate and execute short and long-term care plans, ensuring patients receive comprehensive support. Follow-up Ensure ongoing patient support by following up on referrals and care episodes, monitoring progress, and adjusting care plans as necessary.
